We are conveniently located on Tamarack Avenue in South Windsor with easy access to I-84, I-384, and Route 5. From Buckland Street, turn onto Deming Street. The first left is Tamarack Avenue. Evergreen Endoscopy Center is the third building on the left.
Our regular office hours are Monday through Friday, 7 am – 4 pm. We are also open some Saturdays; please call (860) 644-7336 for those days/times.
At Evergreen Endoscopy Center, we accept most major insurance plans. Please contact your insurance company or you may call our office at (860) 644-7336 to find out if we accept your insurance plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience, we accept cash, checks, Visa, MasterCard, and Discover.
If you have a question related to your bill or insurance, contact our billing specialists at (860) 533-2124. Pay your bill online.
If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be scheduled.
Please coordinate this with your primary care physician in advance. Appointments will be rescheduled if required referrals are not received by the endoscopy center by the time of your visit.

- Eastern Connecticut Health Network, including Manchester Memorial Hospital and Rockville General Hospital
- Francis Hospital & Medical Center

- Trinity Health of New England
- Windham Hospital
In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT!
If it is not a life-threatening emergency, please call your endoscopist’s office.
An important note regarding advance directives:
While under the care of Evergreen Endoscopy Center, any and all advance directives to limit resuscitation, such as Do Not Resuscitate orders (DNR) will be noted, but not honored. In the event of a medical emergency requiring resuscitation, you will be resuscitated without limitations and transferred to the nearest hospital, at which time any advance directives will be made known to the accepting physician.
Please bring a copy of any advance directives you may have on the day of your procedure and we will add this to your record for reference. For more information regarding advance directives, please see Planning for Future Health Care Decisions in Connecticut .
